US Passport Book - Age under 16

Passports issued to individuals when under age 16 can NOT BE RENEWED.  New or first time passport requirements apply to them.

ALL MINOR CHILDREN APPLYING FOR A PASSPORT MUST APPEAR IN PERSON.
The new/first time passport application for minors is required if the minor:

  • Has never been issued a passport
  • Has been issued a passport
                 BUT
  • The passport is lost or stolen
  • The passport is damaged or mutilated
  • The passport is expired                                        

To process a passport application for a child under age16 both parents must appear together and sign or (See #8 below)

INCORRECT OR MISSING INFORMATION MAY RESULT IN REJECTION OF APPLICATION OR SIGNIFICANT DELAY. 

MAIN STEP:  Execution of the US Passport Application
Please take documents 1 to 9 (from the REQUIREMENT list below ) to US Passport Acceptance Facility near you www.iafdb.travel.state.gov/   

Ask the clerk at the Passport Acceptance Facility to execute the passport application.  Then the clerk places the following documents in a sealed envelope addressed to U.S. Passport Agency and return it to you (You must not open or tamper with this envelope). 

  • Application  Form DS-11  
    You  MUST create your own DS-11 by going through the ON LINE process of the Department of State Website.
    Complete and print the form but do not sign this form until you (parents/minor's Legal guardian(s) with the minor) appear before the authorized acceptance agent who must witness your signature(s).

    Tips to Create Your Form DS-11:
    1.  For mailing address write your home/permanent address. 
    2.  For your most recent passport number (if it applies to you & you don't know the number) write nine digits (any numbers) - Please make sure to white it out after you print the application.
    3.  For the date of issue of your most recent passport 
    (if it applies to you & you don't know that date or any other date on the application) write your best guess for the year (for month and day you can alway write January 1).
     
  • Application Form DS-64 (if it applies)
  • Proof of U.S. Citizenship
  • Two Passport Photographs
  • Most Recent  US Passport (If applies) 
  • Evidence of Child's Relationship to Parents/Guardians(See #6 below)
  • Parental Application Permission Documentation (See #7 below)
  • Proof of Departure (Not more than 2 weeks if you don't need a visa for the country of destination and 4 weeks if you need a visa)
  • Authorization Letter
  • Appropriate Fee ($120.00)

The clerk must write applicant's Full Name on the sealed envelope.
Please make sure to make a COPY of the APPLICATION, AUTHORIZATION & TRAVEL ITINERARY for us before going to the Passport Acceptance Facility.

Send us the sealed envelope ASAP (It is time sensitive) and place the copies (application, authorization & Travel It.) and International Passport & Visa Service Request Form (Item 8 from the REQUIREMENT list below) outside the sealed envelope.
  
PLEASE NOTE:  There will be a Significant Delay if you don't send us the above copies because the sealed envelop can be opened ONLY by government passport authorities and not us.  AND we won't be able to process your passport application if we don't have your information. 


Requirements:

1 - U.S. Passport Application Form DS-11 
You will create your own DS-11 by going through the on line process of the Department of State Website. 
 Form DS-64 if replacing a lost/stolen passport (sign and date this from) 

2 - Proof of U.S. Citizenship

  • If born in USA, original/certified copy of your birth certificate issued by the county or state health department or bureau of vital statistics OR a previous passport. To order your birth certificate, GO to www.vitalchek.com
  • If born outside USA, Certificate of Citizenship, Certificate of Naturalization, Consular Report of Birth Abroad OR a previous US passport.

3 - Proof of Identity of Parent(s) or Guardian

  • Valid driver license
  • Photo ID (Issued by DMV)
  • Government (Federal, State, municipal) employee ID card or pass
    The above items should contain your signature AND physical description or photograph that is a good likeness of you.

4 - Two Recent Passport Photographs 
      Photographs must be: 

  •  2 x 2 inches in size, identical, in color
  • taken within the past 6 months and show current appearance
  • full face, front view with a plain white or off-white background
  • between 1 inch and 1 3/8 inches from the bottom of chin to top of the head
  • taken in normal street attire

5.  Most recent US Passport (If it applies)

6.  Evidence of Child's Relationship to Parents/Guardians
     Note: Previous U.S. passports are not acceptable as proof of relationship to the applying parent(s)/guardian(s).
You will need to submit one of the following:

  • Certified U.S. birth certificate (with parents' names); or
  • Certified Foreign Birth Certificate (with parents' names and translation, if necessary); or
  • Report of Birth Abroad (Form FS-240) (with parents' names); or
  • Certification of Birth Abroad ( Form DS-1350) (with parents' names); or
  • Adoption Decree ( with adopting parents' names); or
  • Court Order Establishing Custody; or
  • Court Order Establishing Guardianship.

NOTE: If the parent(s)'/guardian's name(s) is/are other than that on these documents, evidence of legal name change is required.

7.  Proof of Travel
Copy of your travel itinerary or  business letter on your company's letter confirming parent(s) or guardian overseas trip in the next 12-15 days and minor is also traveling.

You have to provide proof of travel document confirming parent(s)/guardian(s) trip overseas in the next 2 weeks (if no visa is required for your trip) and in the next 4 weeks (if you need a visa for the country of your destination)

8. Parental Application Permission Documentation 
    Both parents must appear together and sign OR

 -  One parent appears, signs, and submits second parent's notarized statement of consent authorizing passport issuance for the child (a notarized  Form DS-3053, Statement of Consent: Issuance of a Passport to a Minor Under Age 16 , or a notarized written statement with the same information on a sheet of paper from the non-appearing parent) or

-  One parents appears, signs, and submits primary evidence of sole authority to apply (such as one of the following):

  • Child's certified U.S. or foreign birth certificate (with translation, if necessary) listing only applying parent; or
  • Consular Report of Birth Abroad (Form FS-240) or Certification of Birth Abroad (Form DS-1350) listing only applying parent; or
  • Court order granting sole custody to the applying parent (unless child's travel is restricted by that order); or
  • Adoption decree (if applying parents is sole adopting parent); or
  • Court order specifically permitting applying parent's or guardian's travel with the child; or
  • Judicial declaration of incompetence of non-applying parent; or
  • Death certificate of non-applying parent.

If none of the above documentation is available, the applying parent/guardian should submit a Form DS-3053: Statement of Consent: Issuance of a Passport to a Minor Under Age 14 .

NOTE:  A third-party in loco parent is applying on behalf of a minor under the age of 14 must submit a notarized written statement or affidavit from both parents or guardians authorizing a third-party to apply for a passport. When the statement or affidavit is from only one parent/guardian, the third-party must present evidence of sole custody of the authorizing parent/guardian.

9 -  International Passport & Visa Authorization Letter Should be completed and signed by one parent/guardian. 

 

  • Passport Application Fee - Payable to the U.S. Department of State in the amount of $120.
  • Execution Fee - Payable to the authorized acceptance agent in the amount of $25.
    Please write the minor's full name and date of birth on both checks.
