Requirements:

1 - Your most Recent Passport.
2 - Completed & signed application Form DS-5504
Your  MUST create your own DS-5504 by going through the ON LINE process of the Department of State Website.

3 - The documentation required for the change

4 - Two Recent Passport Photographs 
      Photographs must be: 

•  2 x 2 inches in size, identical, in color
• taken within the past 6 months and show current appearance
• full face, front view with a plain white or off-white background
• between 1 inch and 1 3/8 inches from the bottom of chin to top of the head
  taken in normal street attire

5 - Proof of Travel: Copy of your travel itinerary  OR  a  business letter on your company's letter
     You have to provide proof of your travel overseas in the next 2 weeks (if no visa is required for your trip) and in the next 4 weeks (if you need a visa for the country of your destination).

6 - International Passport & Visa Authorization Letter, Completed & Signed

7 - Completed International Passport & Visa Service Request Form 

NOTE:  Please don't hold on to your application and other paperwork when signed and dated.  Send them to us ASAP (They are time sensitive).
