U.S. Re-Application Form (DS-5504) can be used for making a data correction for a previously issued, currently valid U.S. passport Regardless of the age of the valid passport if the cause is a printing error by Passport Services.
- When there is a change in your identifying information, such as a name-change due to marriage or by court order;
- To extend the validity of certain previously limited passports; or
- To correct a Passport Services’ printing error.
Requirements:
1 - Your most Recent Passport.
2 - Completed & signed application Form DS-5504
You MUST create your own DS-5504 by going through the ON LINE process of the Department of State Website.
3 - Two Recent Passport Photographs
Photographs must be:
- 2 x 2 inches in size
- identical
- taken within the past 6 months and show current appearance
- in color
- full face, front view with a plain white or off-white background
- between 1 inch and 1 3/8 inches from the bottom of chin to top of the head
- taken in normal street attire
4 - Proof of Travel - Copy of your travel itinerary OR A You have to provide proof of your travel overseas in the next 2 weeks (if no visa is required for your trip) and in the next 4 weeks (if you need a visa for the country of your destination).

If it is MORE than one year since the date of issue , you must submit Form DS-82 (or DS-11, if appropriate).
Printing Errors by Passport Services:
If there is a printing error due to Passport Services and your passport was issued less than 30 days ago, please submit the following:
- A letter explaining the specific correction needed and the date by which the corrected passport is needed;
- The passport containing the printing error;
- The documentation required for the correction (see Instruction Page 2 of the form), and
If it has been more than 30 days since your passport was issued, please submit the following:
- The passport containing the printing error
- A completed form DS-5504
You will create your own DS-5504 by going through the on line process of the Department of State Website.
- Two new passport photos
- The documentation required for the correction
NOTE: Please don't hold on to your application and other paperwork when signed and dated. Send them to us ASAP (They are time sensitive).
