You are eligible to add pages to your passport if:
- Your most recent passport is still valid
- You can submit your most recent passport
- Your passport is in a good shape (Not Mutilated)
- You are not making any changes in your identifying information of your passport
Requirements:
1 - Your most Recent Passport.
2 - Completed & signed application Form DS-4085
You MUST create your own DS-4085 by going through the ON LINE process of the Department of State Website.
3 - Proof of Travel - Copy of your travel itinerary OR a business letter on your company's letter.
You have to provide proof of your travel overseas in the next 2 weeks (if no visa is required for your trip) and in the next 4 weeks (if you need a visa for the country of your destination).
4 - International Passport & Visa Authorization Letter, Completed & Signed
5 - Completed International Passport & Visa Service Request Form
NOTE: Please don't hold on to your application and other paperwork when signed and dated. Send them to us ASAP (They are time sensitive).
