U.S. Re-Application Form (DS-5504) should be used for making a data change for a previously issued, currently valid U.S. passport WITHIN one year from the date of issue ONLY:

Requirements:

1 - Your most Recent Passport
2 - Completed & signed application Form DS-5504 
You  MUST create your own DS-5504 by going through the ON LINE process of the Department of State Website.

3 - Two Recent Passport Photographs 
      Photographs must be: 

•  2 x 2 inches in size
• identical
• taken within the past 6 months and show current appearance
• in color
• full face, front view with a plain white or off-white background
• between 1 inch and 1 3/8 inches from the bottom of chin to top of the head
• taken in normal street attire 

4 - Proof of Travel - Copy of your travel itinerary OR A You have to provide proof of your travel overseas in the next 2 weeks (if no visa is required for your trip) and in the next 4 weeks (if you need a visa for the country of your destination).

6 - Completed International Passport & Visa Service Request Form 

If it is MORE than one year since the date of issue, you must apply on application Form DS-82 or DS-11 depending on your circumstances. 

NOTE:  Please don't hold on to your application and other paperwork when signed and dated.  Send them to us ASAP (They are time sensitive).
